Advanced Care Directives

An Advanced Care Directive (ACD) is a formal document that establishes the kind of medical care you want at the end of your life.
Your family must know your wishes. Don’t burden them with difficult decisions upon your incapacitation.

There are many aspects of care to be considered, such as invasive vs non-invasive breathing assistance, CPR, nutrition, hydration, alternative therapies, spiritual beliefs, and ethnic customs.

*I am not an attorney, and one is not required by the
State of Oklahoma.
Oklahoma law states that: (1) an ACD must be signed in the presence of two witnesses.  Witnesses must be 18+ years of age, not related to the signer, and cannot inherit from the signer’s estate, and (2) an ACD is revocable at any time once the signer or a witness notifies the doctor or other healthcare provider.
