This serves as a general guideline. All services are customizable and will be tailored to your specific needs.
EOL Planning
Consultation: $25 for one 30-minute session via phone or online
$40 for one 30-minute session in-person
Planning Session: $300 for 3 hours. Additional sessions are available, if needed, at a rate of $75/hour.
Tours: $100 each
Forms: $250 for Advanced Care Directive, Medical Proxy, or EOL Plan completion
$100 for form review
Notification/Obituary: $150 each
Complete EOL Package: $650 ($840 value, a $190 savings)
Includes one Consultation, one Planning Session, one Tour, one Form, and one Notification OR Obituary. Additional Session hours at a rate of $50/hour ($25/hour savings). Additional forms for $200 each ($50 savings). Add Notification or Obit for $100 ($50 savings).
Funeral Planning
Consultation: $25 for one 30-minute session via phone or online
$40 for one 30-minute session in-person

Officiant: $350 Performs ceremony

Living Funerals

Vigils
Consultation: $20 for one 30-minute session via phone or online
$40 for one 30-minute session in-person
Vigil Planning Session: $250 for 2 hours. Includes Consultation.
Vigil Sitting: $150 for 2 hours, each additional hour $50
Vigil Package: $400 ($500 value, $100 savings)
Includes Consultation, Planning Session, and four (4) hours of Vigil Sitting.
Pet Funerals
Consultation: $15 for one 30-minute session via phone or online
$25 for one 30-minute session in-person
Pet Funeral Planning: $50 per 2-hour session. Includes Consultation.
Officiant: $50
Coordinator: $50 plus cost of supplies, if any are requested
Transport: $20 Delivery to and collection from veterinarian or other disposition facility.
Pet Funeral Package: $75 ($120 value, $45 savings). Includes Consultation, Coordination of event, and Transport.
Terms

Discounts
Packages combining services are available with a discount applied.
Referrals receive a 10% discount on future services, when both parties pay in full for at least one service.
Payment Info
End-of-Life Doula services are not currently covered by Medicare or private insurance.
Each service will be invoiced to be paid within 30 days of service date or death, whichever comes first.
Payment forms accepted are cash, check, and major credit cards.

You have the right to:
Respect for your cultural and spiritual beliefs, values, customs, and preferences

Courteous, responsive, and respectful treatment from healthcare workers
Timely, appropriate, and effective pain management
Refuse care or treatment; refuse to consent
Complain without fear of retaliation or negative consequences to your future treatment
Report and seek quick resolutions to any problems you have with your healthcare, including billing, denied treatment, waiting times, lack of services, or other shortfalls
Safety
You have the right to:
Be cared for in a safe, clean environment
Be free from neglect, misuse, verbal, sexual, mental, and physical abuse at the hands of your providers
Be informed of the need for physical or chemical restraints for your own safety and the safety of others
Be offered access to advocacy services
Know the names of the people caring for you
Privacy and Confidentiality
You have the right to:
Personal privacy
Refuse recordings, videotaping, or other digitally invasive procedures that are not part of your medical care
Medical records, contact information, payment records, and the ability to share them with your consent only except when necessary between practitioners in the act of providing your care
Information
You have the right to:
Be fully informed of your health condition, treatment plan, and services as they change
Be involved in care planning to the extent you wish or are able
Receive information in a way that meets your need for vision, speech, hearing, or cognitive impairment
Designate an agent to assist you in advocating for your wishes
Have your medical and end of life wishes followed
What if death occurs before contracted service?
If death occurs before a service has been provided, monies minus 10% will be refunded to the original payment form.
If payment for service in whole or in part has not been received prior to death, a 10% fee will be invoiced to the estate.
